Case Management
Community Case Management provides services to adults who have developmental disabilities. Case Management Services are provided to help individuals and their families experience growth while fostering independence and integration. Case Management Services include Comprehensive Assessments, developing or assisting in Person-Centered Plans, Referrals, Coordination of health and wellness appointments, Monitoring of activities, and Rep payee services.
Residential Supports
Uplift, Inc. offers a broad variety of residential services and supports to enable a person with developmental disabilities to live successfully in the community. We provide support for people in their own homes, apartments, or homes provided by Uplift. We work closely with the individual seeking support and their families to explore each person’s needs.
Shared Living
Shared living is a unique living option for adults with intellectual disabilities or autism who have section 21 or 29 waiver funding and reside in Maine. It allows the individual the option to be matched with a home provider in the community. The consumer is provided the opportunity to participate with the home provider and their family in all of the day-to-day activities as well as inclusion in the community.
Billing Services
Uplift, Inc. offers billing services for MaineCare under Section 21 & Section 29. All billing is done electronically to the state and we offer several options for billing cycles whether weekly, biweekly, or monthly. We work with providers and MaineCare representatives as necessary; this ensures that we keep up to date on policies and/or any changes.
